Endeavour Psychological Consulting Ltd (Trading as Andrew Stephens Psychologist) takes the storage and handling of your personal data extremely seriously.

The type of personal information I collect and process includes the following:

  • Personal identifiers, contact details and characteristics (e.g., your name and contact details)

  • General administration (e.g., dates and times of your appointments)

  • Financial records (e.g., records of invoices sent to you, payments received, and your payment details)

  • Familial relationships (e.g., emergency contact details)

  • Sensitive information (e.g., information about your health and wellbeing)

  • Notification data

The personal information I process is provided to me directly by you, or by third-parties from whom you have authorised me to collect information (e.g., friends, relatives, employers, or educational institutions), for the purposes of:

  • undertaking psychological assessment and formulating recommendations;

  • providing therapeutic, consultation, or professional supervisory services;

  • complying with my regulatory obligations to maintain accurate records of the psychological services I provide; and

  • billing and complying with my taxation and insurance obligations.

I may share this information with:

  • Organisations providing the electronic communication services through which I communicate with you (e.g., Microsoft, Egress, WhatsApp);

  • Organisations providing billing and financial management services (e.g., PayPal, FreeAgent);

  • Third-parties providing the digital assets through which I will conduct part of your assessment (e.g., Pearson Clinical UK, WPS Online); and

  • Third-parties who have engaged me to provide services to you (e.g., insurance companies or third-party clinics).

Under the UK General Data Protection Regulation (UK GDPR), the lawful bases I rely on for processing this information are:

  • (a) Your consent. You are able to remove your consent at any time. Your information is securely stored on my encrypted electronic hard disk drive and in a secure cloud-based (i.e., Microsoft Office 365) storage system and the secure online storage systems of third-party organisations with whom I must share select information (e.g., your contact and billing details) in order to provide services to you (e.g., PayPal, FreeAgent, Pearson Clinical UK, WPS Online).

I keep your personal information for as long as is necessary to provide the services you have contracted me to provide (e.g., assessment or consultative services), and after that I will typically retain information for a period of seven years in accordance with my insurance obligations. I will then dispose of your information by securely deleting it from my encrypted hard disk drive and cloud-based storage system.

Under data protection law, you have rights including:

  • Your right of access - You have the right to ask me for copies of your personal information. 

  • Your right to rectification - You have the right to ask me to rectify personal information you think is inaccurate. You also have the right to ask me to complete information you think is incomplete. 

  • Your right to erasure - You have the right to ask me to erase your personal information in certain circumstances. 

  • Your right to restriction of processing - You have the right to ask me to restrict the processing of your personal information in certain circumstances. 

  • Your right to object to processing - You have the the right to object to the processing of your personal information in certain circumstances.

  • Your right to data portability - You have the right to ask that I transfer the personal information you gave me to another organisation, or to you, in certain circumstances.

You are not required to pay any charge for exercising your rights. If you make a request, I have one month to respond to you.
